Request a Transcript

Request your Lewis and Clark Transcript

L&C’s Enrollment Center provides official copies of your transcript to any institution or individual you choose at no charge. Please provide the Enrollment Center with a signed written request and your social security number.

How do I request a transcript?

You can request a transcript three different ways:
    • In person. Transcript Request Forms are available in the Enrollment Center on the main campus and at the Community Education Centers. You can also download the form here.
    • By fax or mail. Download the request form and follow the instructions on the form. Fax number is (618) 468-2310. 
      Mail requests to: Lewis and Clark Community College, c/o Enrollment Center, 5800 Godfrey Rd., Godfrey, IL  62035  
    • Online. Online requests require a BlazerNet ID. Log into the My L&C Portal, and refer to BlazerNet in the bottom of the righthand column. Click "Students" then "My Academic Records" and "Request a Transcript."
What information do you need?
  • The name and address of the recipient of the transcript
  • Your name 
  • Date of Birth
  • Student ID or last 4 digits of your SSN
  • Your signature
Will this cost anything?

No. There is no fee for this service. Requests for more than ten official transcripts do require review and approval by the Director, Admissions, Records and Recruitment.  

High School and Other Transcripts

Can I obtain my other college or high school transcripts through L&C?


No, we can only provide copies of your L&C transcript.  For transcripts from other schools, you must contact each institution and have an official transcript sent to L&C.
